ADGL Certificates
Certificates for the Sacraments of Baptism and Confirmation, as well as First Communion, are now available for download for use by parishes in the Anglican Diocese of the Great Lakes. The certificates are available in two formats: Microsoft Word and Acrobat PDF. Directions for using the forms follow.
Word documents
If you have Microsoft Word installed on your computer, we recommend that you download the Word version of the forms. These forms come pre-filled with text fields for "Name," "Date," "Church," etc. To download them,click on the appropriate link below. Open the form in your Word application, and simply over-write these text fields with your own information. (Note: These forms are not locked down, so please use care not to over-write the actual certificate information.)
PDF documents
If you don't have Microsoft Word installed on your computer, or if you prefer to hand-letter your certificates or fill them in on a typewriter, we recommend you download the PDF version of the forms. To download them, simply click on the appropriate link below. Open the form in Acrobat Reader (if Acrobat Reader is not already installed on your computer, click here to download this free program), then print to any computer.
These forms are coming soon in black and white versions for parishes that don’t have access to color printers.
